Résumé :

Objectifs : Le traitement par sclérothérapie a été suggéré comme traitement des malformations vasculaires à flux lent et particulièrement des malformations veineuses.
Ce travail rapporte l’expérience du traitement par échosclérose par polidocanol mousse dans les malformations à flux lent du service de Médecine Vasculaire de Grenoble.
Matériels et méthodes : Il s’agit d’une série continue rétrospective monocentrique de l’ensemble des dossiers des patients présentant une malformation vasculaire symptomatique à flux lent de type veineuse ou veinolymphatique ou complexe de type syndrome de Klippel-Trenaunay (SKT) traité par échosclérose entre janvier 2006 et décembre 2009.
L’indication thérapeutique était toujours préalablement validée par le comité de concertation des malformations vasculaires du Centre Hospitalier et Universitaire de Grenoble.
Toutes les malformations vasculaires étaient classées selon la classification de Hamburg.
Le produit sclérosant utilisé était le polidocanol mousse.
Résultats : 24 patients de 7 à 78 ans ont été traités (19 malformations veineuses, 3 SKT et 2 malformations veino-lymphatiques). Les concentrations de polidocanol utilisées variaient de 0,25 à 3 %. Le nombre moyen de séance était de 2,3.
Après une médiane suivi de 5 mois après la dernière visite, 23 patients sur 24 notaient une diminution de leur douleur ; dans 9 cas, une réduction de plus de 50% du volume initial était obtenue et 58,3% une réduction de moins de 50 % du volume initial. Deux effets secondaires mineurs étaient notifiés.
Conclusions : Le traitement par échosclérose au polidocanol mousse apparaît comme un traitement bien supporté, permettant une amélioration de la symptomatologie des malformations à flux lents sans les risques des produits sclérosants plus agressifs tels que l’éthanol.

Summary :

Aims: Sclerotherapy has been suggested as a treatment for slow-flow vascular malformations and especially venous malformations.
This article reports on our experience of echosclerotherapy using polidocanol foam in slowflow malformations at the Vascular Medicine Department of Grenoble University Hospital.
Materials and methods: This is a single-centre retrospective study of the medical records of a continuous series of all patients presenting with a symptomatic slow-flow venous, or veno-lymphatic or Klippel-Trenaunay-syndrome (KTS)-type complex vascular malformation treated by echosclerotherapy
between January 2006 and December 2009.
The therapeutic indication was always validated beforehand by the Grenoble University Hospital multidisciplinary committee for vascular malformations.
All of the vascular malformations were classified according to the Hamburg Classification.
Polidocanol foam was the sclerosing agent used.
Results: 24 patients from 7 to 78 years were treated (19 venous malformations, 3 KTS and 2 veno-lymphatic malformations). The concentrations of polidocanol used ranged from 0.25 to 3%. The mean number of sessions was 2.3.
After a median follow-up of 5 months after the last visit, 23 patients of the 24 reported a decrease in pain; in 9 cases, a reduction of more than 50% in the initial volume was achieved, and in 58.3% a reduction of less than 50% in the initial volume. Two cases of minor adverse side effects were reported.
Conclusions: Echosclerotherapy with polidocanol foam appears to be well tolerated, improves the symptoms of slow-flow malformations and does not carry the risks of more aggressive sclerosing agents such as ethanol.
